Job summary

The Wonderful Company seeks a Staff Physician (IM/FM) for a 100% outpatient practice in Lost Hills and Delano, CA. This full-time role focuses on prevention, obesity, diabetes and cardiometabolic care, working with a multidisciplinary team to improve patient outcomes.

Qualifications

• MD or DO from an accredited medical school.
• Board-certified in Family Medicine or Internal Medicine preferred.
• Current or eligible for unrestricted California medical licensure.
• Current DEA registration or eligibility to obtain registration.
• Current BLS certification or ability to obtain prior to start date.
• Experience providing outpatient primary care strongly preferred.
• Experience managing patients with obesity, diabetes, and other cardiometabolic conditions preferred.

Responsibilities

• Provide compassionate, high-quality, evidence-based primary care, including preventive care, chronic disease management, and evaluation and treatment of acute conditions.
• Diagnose, treat, and manage obesity and obesity-related conditions, including diabetes, hypertension, dyslipidemia, and other cardiometabolic diseases.
• Develop individualized treatment plans and partner with patients to support sustainable health improvements.
• Manage an empaneled patient population and coordinate care across the multidisciplinary clinical team.
• Collaborate with physicians, APPs, behavioral health, physical therapists, health coaches, and other care team members.
• Provide clinical guidance to care team members to support safe, coordinated patient care.
• Participate in clinical quality improvement and population health initiatives.
• Review clinical and population health data to identify opportunities to improve patient outcomes.
• Coordinate referrals and follow-up care to promote continuity of care.
• Support onsite, mobile, and community-based efforts to improve engagement and access.
• Maintain accurate medical documentation and adhere to standards.
• Provide care in a 100% outpatient practice, seeing about 16 patients per day.
• Supervision of, and collaboration with, non-physician providers.
