Job summary

The Wonderful Company seeks a Staff Physician (IM/FM) for an onsite 100% outpatient role in Lost Hills/Delano, CA. Focus on preventive care, obesity, diabetes, and cardiometabolic health with a multidisciplinary team and relocation assistance.

Responsibilities include chronic disease management, patient-centered care, and coordination with specialists and care team members. Board certification preferred; California licensure required or eligible.

Qualifications

• MD or DO degree from an accredited medical school.
• Board-certified in Family Medicine or Internal Medicine preferred.
• Current California medical licensure or eligibility.
• Current DEA registration or eligibility to obtain.
• Current BLS certification or ability to obtain prior to start date.
• Experience providing outpatient primary care strongly preferred.
• Experience managing patients with obesity, diabetes, and cardiometabolic conditions preferred.

Responsibilities

• Provide compassionate, high-quality outpatient primary care, including preventive care and chronic disease management.
• Diagnose, treat, and manage obesity and cardiometabolic diseases.
• Develop individualized treatment plans and support sustainable health improvements.
• Manage an empaneled patient population with coordinated care across the team.
• Collaborate with physicians, APPs, behavioral health, and other care team members.
• Provide clinical guidance to care team to ensure safe, coordinated care.
• Participate in quality improvement and population health initiatives.
• Review data to identify opportunities to improve patient outcomes.
• Coordinate referrals and follow-up care for timely access.
• Support onsite, mobile, and community-based patient engagement efforts.
