Job summary

True North Consultants, Inc. seeks a Family Nurse Practitioner for an outpatient primary care setting on Long Island. Flexible full-time or part-time hours, no call, Monday–Friday. Compensation ranges from $200K to $300K annually.

The ideal candidate has NYS RN/NP licenses, BLS, board certification, 8+ years of NP experience, and a patient-centered, entrepreneurial approach to growing a local practice. EMR proficiency is preferred.

Qualifications

• NYS RN and NP license required.
• BLS certification required.
• Board certified NP.

Responsibilities

• Serve as a key clinical provider in the delivery of high-quality care.
• Provide direct patient care in an outpatient primary care setting, including preventive, acute, and chronic disease management.
• Refer patients to appropriate specialty services as needed.
• Provide clear communication and education to patients regarding diagnoses, treatment options, and preventive strategies.
• Maintain thorough and accurate documentation within the EMR system.
• Collaborate with referring physicians, specialists, and care teams to ensure seamless patient care.
• Provide clinical oversight to nurses, technologists, and support staff within the practice.
• Stay compliant with HIPAA, NYS DOH, CMS, and relevant payer guidelines.
• Participate in population health initiatives and support ongoing quality improvement projects.
• Contribute to the development of a sustainable patient panel aligned with practice growth objectives.
