Job summary

A well-established, physician-led group on Long Island is seeking a Family Nurse Practitioner for outpatient primary care. 100% outpatient with no call, flexible hours, and a compensation range of $200K-$300K per year.

Qualifications

• Deliver outpatient primary care to patients including preventive, acute, and chronic disease management.
• Refer patients to appropriate specialty services as needed.
• Provide clear communication and education to patients regarding diagnoses, treatment options, and preventive strategies.
• Maintain thorough and accurate documentation within the EMR system.
• Collaborate with referring physicians, specialists, and care teams to ensure seamless patient care.
• Provide clinical oversight to nurses, technologists, and support staff within the practice.
• Stay compliant with HIPAA, NYS DOH, CMS, and relevant payer guidelines.
• Participate in population health initiatives and support ongoing quality improvement projects.
• Contribute to the development of a sustainable patient panel aligned with practice growth objectives.

Responsibilities
