Job summary

Zaddy Solutions is seeking a Family Nurse Practitioner for 100% outpatient, physician-led group on Long Island. The role focuses on high-quality primary care with preventive, acute, and chronic disease management in an outpatient setting.

Flexible hours (full time or part time), Monday–Friday, no on-call requirements, with a highly competitive compensation package of $200K-$300K per year and comprehensive benefits.

Qualifications

• BSN and MSN from an accredited nursing school.
• NYS RN & NP license, BLS, Board Certified.
• 8+ years experience as a NP.
• Strong diagnostic, communication, and interpersonal skills.
• Entrepreneurial mindset with ability to grow patient base.
• Experience with EMRs and value-based care metrics is a plus.

Responsibilities

• Provide direct outpatient primary care including preventive, acute, and chronic disease management.
• Refer patients to appropriate specialty services.
• Document thoroughly in the EMR system.
• Collaborate with referring physicians, specialists, and care teams.
• Provide clinical oversight to nurses, technologists, and support staff.
• Stay compliant with HIPAA, NYS DOH, CMS, and payer guidelines.
• Participate in population health initiatives and quality improvement.
• Develop a sustainable patient panel aligned with practice growth objectives.
• Serve as a key clinical provider delivering high-quality care.
