Job summary

ExcelliSearch, Inc. is seeking a Family Nurse Practitioner for an outpatient, 100% primary care role on Long Island. Flexible hours with full-time or part-time options and no on-call.

You will deliver preventive, acute, and chronic care, coordinate referrals, and maintain EMR documentation in a collaborative, physician-led group. The ideal candidate has 8+ years as an NP with NYS RN/NP licensure, BSN/MSN, and a patient-centered, entrepreneurial approach to grow a thriving practice.

Qualifications

• BSN and MSN from an accredited nursing school.
• NYS RN & NP license, BLS, Board Certified.
• Strong diagnostic, communication, and interpersonal skills.
• Team-oriented with a patient-centered approach to care.
• Proven ability to grow a local clinical practice.
• Entrepreneurial mindset to grow and manage a thriving patient base.
• Familiarity with EMRs and value-based care metrics is a plus.
• 8+ years experience as a NP

Responsibilities

• Serve as a key clinical provider delivering high-quality care.
• Provide direct patient care in outpatient primary care setting, including preventive, acute, and chronic disease management.
• Refer patients to appropriate specialty services as needed.
• Provide clear communication and education to patients regarding diagnoses, treatment options, and preventive strategies.
• Maintain thorough and accurate documentation within the EMR system.
• Collaborate with referring physicians, specialists, and care teams to ensure seamless patient care.
• Provide clinical oversight to nurses, technologists, and support staff within the practice.
• Stay compliant with HIPAA, NYS DOH, CMS, and relevant payer guidelines.
• Participate in population health initiatives and support ongoing quality improvement projects.
• Contribute to the development of a sustainable patient panel aligned with practice growth objectives.
