Job summary

Top Echelon LLC seeks a Family Nurse Practitioner for outpatient primary care on Long Island, NY. The provider will deliver preventive, acute, and chronic care in a 100% outpatient setting, collaborating with a physician-led team.

NYS RN & NP licensure, BSN/MSN, and 8+ years of NP experience are required. The position offers flexible hours with full-time or part-time options, no on-call, and a compensation range of $200k–$300k per year.

Qualifications

• BSN and MSN from an accredited nursing school.
• NYS RN & NP license, BLS, Board Certified.
• Strong diagnostic, communication, and interpersonal skills.
• Team-oriented with a patient-centered approach to care.
• Proven ability to grow a local clinical practice.

Responsibilities

• Serve as a key clinical provider in the delivery of high-quality outpatient care.
• Provide direct patient care in preventive, acute, and chronic disease management.
• Refer patients to appropriate specialty services as needed.
• Communicate diagnoses, treatment options, and preventive strategies to patients.
• Maintain thorough documentation in the EMR system.
• Collaborate with physicians, specialists, and care teams for seamless care.
• Provide clinical oversight to nurses, technologists, and support staff.
• Stay compliant with HIPAA, NYS DOH, CMS, and payer guidelines.
• Participate in population health initiatives and quality improvement.
• Contribute to development of a sustainable patient panel.
