The candidate should hold NYS RN & NP licenses with BLS and board certification, have BSN/MSN degrees, and 8+ years of NP experience, with strong diagnostic and communication skills.

Qualifikationen

• RN & NP license in NYS, BLS, Board Certified
• BSN and MSN from an accredited nursing school
• 8+ years experience as a NP
• Strong diagnostic, communication, and interpersonal skills
• Team-oriented with patient-centered approach
• Entrepreneurial mindset with ability to grow and manage patient base
• Familiarity with EMRs and value-based care metrics is a plus

Aufgaben

• Serve as a key clinical provider in the delivery of high-quality care.
• Provide direct patient care in an outpatient primary care setting, including preventive, acute, and chronic disease management.
• Refer patients to appropriate specialty services as needed.
• Provide clear communication and education to patients regarding diagnoses, treatment options, and preventive strategies.
• Maintain thorough and accurate documentation within the EMR system.
• Collaborate with referring physicians, specialists, and care teams to ensure seamless patient care.
• Provide clinical oversight to nurses, technologists, and support staff within the practice.
• Stay compliant with HIPAA, NYS DOH, CMS, and relevant payer guidelines.
• Participate in population health initiatives and support ongoing quality improvement projects.
• Contribute to the development of a sustainable patient panel aligned with practice growth objectives.
