Job summary

Stony Brook University’s Endocrinology and Metabolism Division seeks an outpatient Diabetes Nurse Practitioner or Physician Assistant to work with physicians and nurses in patient-centered diabetes care.

The selected candidate will adhere to standards of care, manage diabetes treatment plans, and participate in education and counseling for patients and families, collaborating with the supervising physician. Start date ASAP.

Qualifications

• Active NYS RN license and NYS NP license or PA license required.
• Master’s degree in Nursing or Physician Assistant Studies required.
• AANP/ANCC (NP) or NCCPA (PA) certification preferred.
• Eligible to sit for the Certified Diabetes Educator (CDE) exam within two years.

Responsibilities

• Adhere to standards of care in diabetes management and develop, review and revise treatment plans.
• Assist physician with interventional treatments.
• Respond to patient messages and perform callbacks for reported responses.
• Refer to other providers and order/evaluate appropriate non-invasive tests.
• Instruct and counsel patients and families on diabetes self-management.
• Other duties as assigned.
