Job summary

Fresenius Medical Care is seeking a professional registered nurse at CAP 2 level to join the outpatient team in California. This role supports assessment, planning, intervention and evaluation for a defined patient group, coordinating care with RNs, LVNs/LPNs and patient care technicians.

The RN CAP 2 works within the End Stage Kidney Disease program, contributing to patient education, quality improvement and facility operations, with opportunities for leadership and cap completion through the

Qualifications

• RN CAP 2 entry level into the Clinical Advancement Program (CAP) or promoted from RN CAP 1.
• Minimum of 2 years of nephrology nursing experience.
• Graduate of an accredited School of Nursing and current state licensure.

Responsibilities

• Provide and coordinate clinically competent care including assessment, planning, intervention and evaluation for assigned patients.
• Document patient data and care decisions in the medical record and modify treatment plans as needed.
• Delegate and supervise tasks to RNs, LVN/LPNs, and Patient Care Technicians as appropriate.
• Educate patients and families on treatment options and post-care requirements.
