Job summary

Legend Oaks of Fort Worth is seeking a licensed Occupational Therapist or Licensed Occupational Therapy Assistant to serve as Outpatient Coordinator, building relationships with physicians, office managers, referral coordinators, and community members to discuss patient care and referrals.

The role includes delivering therapy to older adults in SNF and in the community, coordinating outreach events, and collaborating with the Director of Rehab to optimize therapy services.

Qualifications

  • Licensed OT or COTA with current license.
  • 1 year of experience in SNF and/or home health preferred.
  • Experience in healthcare marketing is optional but preferred.
  • Strong interpersonal skills and ability to build rapport quickly.

Responsibilities

  • Build and strengthen relationships with current and prospective referral sources through face-to-face meetings.
  • Identify and market to local primary and specialty physicians to address outpatient therapy needs.
  • Assist the Director of Rehab with coordinating community events or specialty marketing campaigns.
  • Manage outpatient therapy schedule to establish and maintain POC and coordinate with other therapists to reschedule missed visits.
  • Provide therapy services to long-term care, skilled in-patient, and outpatients within the facility.
  • Provide therapy services in outpatients' homes, group homes, ALF/ILF.

Legend Oaks of Fort Worth, TX, is seeking a licensed Occupational Therapist or Licensed Occupational Therapy Assistant to serve as our Outpatient Coordinator to build and strengthen relationships with current and prospective outpatient referral sources by completing face-to-face meetings with physicians, office managers, referral coordinators, and individuals in the community. You will educate these individuals on the services our facility offers, discuss patient care, referrals, and thank them for existing referrals. As an Evaluating Therapist, you will be treating older adults in our skilled nursing facility as well as in the community.

Key Responsibilities:
  • Build and strengthen relationships with current and prospective referral sources by completing face-to-face meetings with physicians, office managers, referral coordinators, and individuals in the community.
  • Identify and market to local primary and specialty physicians to address their patient's outpatient therapy needs.
  • Assist the Director of Rehab with coordinating community events, public relations campaigns, or specialty marketing campaigns
  • Manage outpatient therapy schedule to establish and maintain POC as well as coordinate with other therapists to reschedule any missed visits.
  • Provide therapy services to long-term care, skilled in-patient, and outpatients within the facility.
  • Provide therapy services in the outpatients' homes, group home, ALF/ILF
Requirements:
  • Licensed OT or COTA
  • Experience in marketing in healthcare is optional, but preferred
  • Exceptional interpersonal skills, including the ability to quickly build rapport
  • Outgoing and personable
  • Willingness to become involved in local community events
  • Minimum of 1 year experience in SNF and/or home health
