Job summary

Community House Mental Health Agency in Seattle is seeking a FHARPS Case Manager to join a care team delivering outpatient behavioral health services and community support within the diversion program. You will provide treatment and care coordination based on client needs and the treatment plan, with progress notes and reviews for your caseload.

This full-time role offers a Tue-Sat schedule (9:00am–4:00pm, 35 hours/week) and requires collaborating with internal/external teams, coordinating

Qualifications

  • Bachelor’s degree in social science or related field.
  • 2+ years experience serving adults with severe mental illness and/or in diversion services.
  • Certified Peer Counselor credential acceptable and preferred for FHARPS program.
  • Pass criminal background check before hire.
  • First Aid/CPR certification within 30 days of hire.
  • Valid license for 1 year with clean driving record.

Responsibilities

  • Serve as Primary Case Manager for assigned caseload, coordinating client needs and services; ensure plans and documentation are up-to-date.
  • Coordinate with internal/external departments for referrals and intake; develop individualized plans.
  • Collaborate with care team to ensure access to treatment, support, and appointments; arrange transportation as needed.
  • Outreach to clients in homes or community settings to assess stability and provide interventions.
  • Ensure trauma-informed care principles: safety, choice, trust, collaboration, empowerment.
  • Coordinate medication access following the seven rights; document appropriately.
  • Document interactions in EMR; maintain required program documentation.
