Job summary

Alcott is seeking an Outpatient Case Manager in Culver City, CA to provide comprehensive case management for adults with serious mental illness. You will collaborate with clinicians and community partners to help clients access resources, develop living skills, and navigate recovery through compassionate, person-centered care.

Responsibilities include coordinating medical follow-ups, supporting housing stability, and ensuring timely documentation in line with LAC-DMH and Medi-Cal standards.

Qualifications

• Bachelor's degree in Psychology, Social Work, Counseling, or a related behavioral health field required.
• Experience with LAC-DMH documentation/procedures preferred.
• Military-experienced candidates encouraged to apply.

Responsibilities

• Coordinate medical follow-up and support clients with co-occurring medical/behavioral health needs.
• Provide life skills training promoting independence and recovery.
• Assist clients in securing stable housing and coordinating housing resources.
• Connect clients with community-based services, benefits, and resources.
• Provide crisis intervention and stabilization services.
