Job summary

Alcott is seeking an Interim Housing Case Manager in Culver City, CA. This on-site role involves managing a caseload in an interim housing program, assessing client needs, connecting services, and advocating for stable housing, with a trauma-informed and harm-reduction approach.

The position requires experience with homeless populations, a relevant degree or equivalent experience, and the ability to coordinate with medical and housing services. Salary ranges from $24 to $26 per hour, DOE.

Qualifications

• Bachelor’s degree in social work, psychology, or related behavioral health field or two years of relevant experience with HS diploma/GED
• Experience working with individuals experiencing homelessness preferred
• Familiarity with housing navigation, crisis response, and public benefits systems
• Strong communication, problem solving, and advocacy skills

Responsibilities

• Meet with new clients within 24 hours of program entry to complete intake and orientation
• Build rapport using trauma-informed and harm reduction approaches
• Complete intake documentation within required timeframes and coordinate room assignments with site staff
• Confirm medication intake with medical team and support continuity of care
• Conduct weekly check-ins with clients and document progress in CHAMP
• Assist clients in obtaining IDs, benefits paperwork, and birth certificates
• Monitor mental health symptoms, substance use, and medication adherence
• Schedule and coordinate transportation for medical and housing appointments
• Collaborate with team and attend multidisciplinary meetings for coordinated care
• Develop and update individualized care plans aligned with client goals
