Job summary

Alcott is seeking a Clinical Case Manager – Interim Housing to provide evidence-based clinical services to adults transitioning from correctional settings into Watts, CA community housing. The role requires a Master’s degree in a mental health field and active BBS licensure within 90 days of hire.

You will conduct intake assessments, develop individualized treatment plans, and deliver 1:1 counseling as well as group sessions, while coordinating referrals and maintaining EHR documentation.

Qualifications

• Master’s degree in Social Work, Marriage and Family Therapy, Counseling, or a related mental health field.
• Active BBS registration as ASW, AMFT, or APCC within 90 days of hire.
• Experience in substance use treatment, corrections, interim housing, or community mental health settings is preferred.
• Bilingual Spanish/English candidates strongly encouraged to apply.

Responsibilities

• Provide professional, evidence-based clinical services to adults in Alcott’s Interim Housing program.
• Conduct comprehensive intake assessments and develop individualized, client-centered treatment plans.
• Facilitate 1:1 counseling and psychoeducational group sessions; monitor client progress and adjust plans.
• Coordinate referrals to medical, mental health, and substance use treatment providers; connect clients to community resources.
• Maintain accurate documentation in the electronic health record and ensure compliance with standards.
