Job summary

Alcott is seeking a Clinical Case Manager – Interim Housing to provide evidence-based clinical services to adults transitioning from correctional settings into Watts, CA community housing. The role requires a Master’s degree in a mental health field and active BBS licensure within 90 days of hire.

You will conduct intake assessments, develop individualized treatment plans, and deliver 1:1 counseling as well as group sessions, while coordinating referrals and maintaining EHR documentation.

Qualifications

  • Master’s degree in Social Work, Marriage and Family Therapy, Counseling, or a related mental health field.
  • Active BBS registration as ASW, AMFT, or APCC within 90 days of hire.
  • Experience in substance use treatment, corrections, interim housing, or community mental health settings is preferred.
  • Bilingual Spanish/English candidates strongly encouraged to apply.

What You’ll Do: The Impact You’ll Make
As a Clinical Case Manager, you will provide professional, evidence-based clinical services to adults residing in Alcott’s Interim Housing program, supporting their transition from correctional facilities into community living. You will conduct comprehensive intake assessments and develop individualized, client-centered treatment plans. You will facilitate individual counseling and psychoeducational group sessions, monitor client progress, and adjust treatment plans to support measurable recovery goals. You will deliver evidence-based interventions grounded in harm reduction and trauma-informed care, provide 1:1 counseling to strengthen coping skills and readiness for change, and respond to crisis situations within housing sites and community settings. You will coordinate referrals to medical, mental health, and substance use treatment providers, connect clients to community resources and recovery supports, and collaborate with multidisciplinary teams to ensure integrated care. You will maintain accurate and timely documentation in the electronic health record, participate in case conferences and staff meetings, and ensure compliance with agency standards and funder requirements.

What You’ll Bring: Your Qualifications
Education and Licensure
A Master’s degree in Social Work, Marriage and Family Therapy, Counseling, or a related mental health field is required. Active BBS registration as an ASW, AMFT, or APCC is required within 90 days of hire. Experience providing clinical services in substance use treatment, corrections, interim housing, or community mental health settings is preferred. Military-experienced candidates are encouraged to apply.

Strong crisis intervention, assessment, and documentation skills are required. Familiarity with harm reduction, trauma-informed care, and motivational interviewing is highly desirable. Additional Requirements
Local travel between agency sites is required. A valid California Driver’s License, clean driving record, and insurability under agency policy are required.
