Job summary

Santa Fe Recovery is seeking a dedicated Case Manager to assess clients, develop and monitor Individual Service Plans, and connect clients with community resources to promote independence and continuity of care.

You will document, coordinate with families and providers, maintain HIPAA-compliant records, and support trauma-informed, person-centered services in a collaborative clinical team.

• Attention to detail in documentation and records.
• Maintain professional relationships at all levels within the organization.
• Familiarity with mental health, housing, employment, and other community resources.
• Unrestricted NM Driver’s License.

Responsibilities

• Triage clients to determine level of Case Management care.
• Follow up with clients to ensure service goals are met.
• Educate and connect clients to resources related to Individual Service Plans.
• Maintain positive relations with community services.
• Liaise with family, friends, partners, and outside resources to ensure continuity of care.
• Promote resources and skills training for person-centered care.
• Work with clinical team to address daily concerns and intervene when needed.
• Assist clients in developing an Individual Service Plan.
• Uphold participant confidentiality per policies and laws.
• Maintain case files and documentation per agency policies.
• Identify and maintain relationships with new employment, housing, medical, and community resources.
• Educate clients on scheduling appointments related to services.
• Ensure timely completion of documentation.
• Perform other duties as assigned.
