Qualifications

• High School Diploma, GED or equivalent.
• 2+ years experience in a patient care or public health setting.
• Current JPS CPR/BLS Certification and valid Driver's License.
• OR 2+ years experience as a Medical Assistant with CPR/BLS and license.
• OR 30–60 college hours in a health field with CPR/BLS and license.

Responsibilities

• Screens referred patients for additional services in Outpatient Case Management.
• Assists patients in accessing health services and locating a medical home.
• Establishes trusting relationships with patients and families.
• Uses electronic resources to collect, report, and document data.
• Supports follow-up to link patients to primary care and screenings.
• Acts as a point of contact for patients and conducts outreach.
• Performs face-to-face contacts at home, hospital, or medical home.
• Schedules follow-up visits and coordinates care management activities.
• Performs non-clinical case management duties including transportation coordination and translation.
