Job summary

NewYork-Presbyterian is seeking a Care Management Coordinator – RN for Ambulatory care, days, to manage high-risk patients in the local community, improving chronic illness management and reducing unnecessary ER visits.

The role requires BSN and NYS RN licensure, with 5+ years healthcare experience and 2+ years case management preferred. On-site, full-time at NYP/Columbia area; strong EMR, Word, Excel skills needed; bilingual English/Spanish a plus.

Qualifications

• 5+ years of healthcare experience (Required)
• 2+ years of case management experience in inpatient, HMO, Insurance, or other outpatient setting (Preferred)
• Experience with electronic medical records (Required)
• Computer proficiency in Word and Excel (Required)
• Excellent interpersonal skills (Required)
• Ability to problem solve and prioritize (Required)
• Flexibility and ability to work in fast-paced, changing environment (Required)
• Bilingual English and Spanish (Preferred)

Responsibilities

• Conduct case-finding, assessments, education, counseling and other interventions to patients targeted through chronic disease registries.
• Assist patients in navigating NYP service delivery system and other services to overcome barriers to self-management and adherence.
• Connect patients and their families to community resources and ensure the proper continuation of quality care.
