Job summary

JPS Health Network is seeking a Case Management Coordinator for Outpatient Case Management on the Day shift at John Peter Smith Hospital in Fort Worth, TX. This role supports case management goals by conveying program information, collecting data, and performing clerical duties to assist Nurse Case Managers and Social Workers.

Responsibilities include patient screening for services, coordinating care transitions, and promoting health literacy while working with a clinical team in multiple sites

Qualifications

• High School Diploma, GED or equivalent.
• 2+ years experience in patient care or public health setting.
• Current CPR/BLS Certification and must be maintained throughout employment.
• Current Driver’s License in good standing.
• Between 30-60 college semester hours completed in a health-related field.

Responsibilities

• Screens referred patients for additional services provided by the Outpatient Case Management Department.
• Supports patients in accessing health-related services and a medical home.
• Establishes trusting relationships with patients and families.
• Uses electronic resources to collect, report and document data for case management.
• Performs outreach to link patients to primary care and preventive services.
• Acts as a point of contact for patients and families for follow-up activities.
