Job summary

VHC Health is seeking an Registered Nurse (RN) or MSW Case Manager for our Outpatient Clinic. The role focuses on coordinating care through assessment, planning, facilitation, and advocacy to meet patients' health needs while ensuring cost-effective outcomes.

Responsibilities include maintaining a coordinated continuum of care with the interdisciplinary team, patients and families, and securing necessary approvals with payers. Prioritizes discharge planning and community resource coordination.

Qualifications

• Graduation from an accredited school of social work, nursing, in a related field or equivalent is required.
• Five years of experience in the healthcare field is preferred.
• Experience in case management is preferred.

Responsibilities

• Collaborative process of assessment, planning, facilitation, and advocacy for options and services to meet health needs, promoting quality, cost-effective outcomes.
• Ensure identified and referred patients have a coordinated continuum of care per JCAHO and hospital guidelines; liaison for the interdisciplinary team, patient, and family for timely, clinically appropriate care planning.
• Maintain communications with payers at admission and throughout hospitalization to secure approvals and clarify benefits.
• Facilitate retrospective reviews and approvals for care as necessary, and implement and coordinate discharge plans utilizing community resources.
