Qualifications

• Licensed Nurse Practitioner in the state of employment.

• Clinical experience in substance use rehabilitation preferred.
• Experience documenting into an electronic medical record.

Responsibilities

• Perform medical evaluations, histories, and physical exams before starting medication.
• Manage ongoing treatment including prescribing and updating methadone treatment plans.
• Complete required documentation and medical orders in state and federal compliance.
• Maintain patient case management for chronic health issues and medications.
• Coordinate referrals and continuum of care with other healthcare professionals.
• Educate patients and families on medications, side effects, and nutrition.
