Qualifications

• Nurse Practitioner with an active, unrestricted license to practice in the state of practice.
• DEA Certificate of Registration with ability to schedule 2, 2N, 3, 4, 5.
• Must have or be eligible for Medicaid/Medicare enrollment. Additional state requirements may apply.

Responsibilities

• Evaluate and manage patients with opioid use disorder in outpatient clinic settings.
• Prescribe appropriate medications and treatments as indicated by patient assessment.
• Complete medical documentation per Crossroads standards.
• Collaborate with clinic staff to provide holistic care to patients, including referrals to other medical providers and community services.
• Will perform injections.
