Virtua Health in New Jersey seeks an experienced RN or Masters prepared Social Worker to join Per Diem Outcomes Management. You will perform comprehensive assessments, develop care plans, and document in the EMR and case management systems.

Qualifications

• RN or Masters prepared Social Worker is required.
• Excellent verbal and written communication, problem solving, and critical thinking.
• Experience in case management or homecare coordination is preferred.
