Virtua Health is seeking a Per Diem Outcomes Manager to coordinate patient care across five hospital campuses in New Jersey. Requires weekday/weekend availability 8:30 a.m. to 5:00 p.m.; two special days per year.

Performs care coordination, documentation, and interdisciplinary collaboration to enhance quality and cost-effective care. Responsibilities include assessing needs, communicating with patients and families, maintaining EMR records, and supporting continuous improvement while ensuring

Qualifications

• Minimum 3 years clinical nursing (RN) experience or Social Work.
• Minimum of 1 year Case Management or Homecare Coordination in an Acute Care setting.
• Excellent verbal and written communication, problem solving and critical thinking.

Responsabilités

• Care Coordination - completes assessments and plans effectively to meet patient needs.
• Problem resolution and patient/family communication; escalate as appropriate.
• Documentation - complete EMR and case management documentation.
• Metrics - contribute to dashboards and organizational success.
• Participates in improvement activities and compliance with regulations.
