Qualifications

• Minimum 3 years clinical nursing (RN) experience or Social Work.
• Minimum of 1 year Case Management or other interdisciplinary experience including Homecare Coordination in an Acute Care setting; understanding of Medicare, Medicaid, managed care and discharge planning.

Responsibilities

• Care Coordination – assesses and plans to meet patient needs, monitors length of stay and promotes efficient resource use.
• Documentation – complete documentation of plan of care in EMR and case management system.
• Metrics – accountable to job-specific goals and dashboards contributing to organizational success.
• Organizational improvement – participates in patient satisfaction, Six Sigma committees, and cross-team initiatives.
• Compliance – understands and applies federal/state requirements and reports compliance issues.
