Job summary

Moraga Post Acute is seeking a compassionate Occupational Therapist (OTR) for a PRN position. The starting rate is $58–$63+ per hour and you will join our rehabilitation team to help residents maximize independence and quality of life.

Responsibilities include evaluating residents, creating treatment plans, providing OT services for ADLs, supervising COTAs, coordinating with the interdisciplinary team, and contributing to discharge planning.

Qualifications

• Current California OTR license required.
• Strong clinical, communication, and organizational skills.
• Able to work effectively within a multidisciplinary team.
• Commitment to resident-centered care.

Responsibilities

• Evaluate residents and develop individualized treatment plans based on functional needs.
• Provide skilled OT services to improve independence in activities of daily living (ADLs).
• Supervise and collaborate with COTAs in accordance with state regulations.
• Coordinate care with physicians, nursing staff, residents, families, and the interdisciplinary team.
• Participate in care conferences, rehabilitation meetings, and discharge planning.
• Educate residents and caregivers on treatment goals, home exercise programs, adaptive techniques, safety awareness, and fall prevention.
• Recommend adaptive equipment and durable medical equipment (DME) to support resident independence.
• Maintain accurate, timely, and compliant clinical documentation.
• Contribute to quality improvement initiatives and staff education programs.
