Job summary

Diablo Valley Post Acute in Concord, CA is hiring an Occupational Therapist (OTR) to join our in-house rehabilitation team. Full-time, part-time, and PRN opportunities with pay $59–$66+ per hour DOE, flexible scheduling.

We offer mentorship, ongoing professional development, and collaborative care with physical and speech therapists, nurses, and support staff to help residents achieve independence.

Qualifications

• Current California Occupational Therapist (OTR) license in good standing.
• Strong communication and interpersonal skills.
• Passion for rehabilitation and resident-centered care.

• Commitment to providing high-quality clinical care.

Responsibilities

• Perform comprehensive occupational therapy evaluations.
• Develop and implement individualized treatment plans.
• Provide evidence-based interventions focused on improving functional independence and quality of life.
• Address activities of daily living, mobility, coordination, strength, cognition, and other functional needs.
• Monitor resident progress and modify treatment plans as appropriate.
• Educate residents, families, and caregivers on therapeutic strategies and safety.
• Collaborate with the interdisciplinary team to coordinate care and discharge planning.
• Maintain accurate, timely, and compliant clinical documentation.
