Continuum Therapy Partners is seeking a Certified Occupational Therapy Assistant to support OT-led patient care at Trinity Care Center in Farmington, MN. The role involves implementing treatment plans, documenting progress, and educating patients on adaptive equipment and home programs.

The ideal candidate holds an Associate Degree in Occupational Therapy Assistant, an active MN COTA license, and NBCOT certification, with 1 year of COTA experience preferred.

Qualifications

• Associate degree is required.
• Current, unencumbered Minnesota COTA license; NBCOT certification required.
• Minimum 1 year of COTA experience preferred.

Responsibilities

• Implements individualized treatment plans under the supervision of an Occupational Therapist.
• Documents patient progress and reports changes in patient status to the supervising OT.
• Educates patients on adaptive equipment and home exercise programs.

Certified Occupational Therapy Assistant Requirements & Responsibilities:

• Associate Degree in Occupational Therapy Assistant Program required

• Current, unencumbered state COTA License; NBCOT Certification required

• 1 year of previous experience as a Certified Occupational Therapy Assistant preferred
