Job summary

Continuum Therapy Partners, LLC is seeking a Certified Occupational Therapy Assistant to join our Sturgeon Bay health services team. The role involves working under the supervision of an Occupational Therapist to implement treatment plans and monitor patient progress, with emphasis on adaptive equipment education and home exercise programs.

The position is PRN with daytime shifts at Sturgeon Bay Health Services, WI.

Qualifications

• Associate degree obtained and licensure as COTA required
• NBCOT certification active and unencumbered license required
• 1 year of experience preferred but not required to apply
• Assists with implementing therapy plans under OT supervision
• Keeps patient progress notes and communicates changes to supervising OT
• Educates patients on adaptive equipment and home exercise programs

Responsibilities

• Implements individualized treatment plans under the supervision of an Occupational Therapist
• Documents patient progress and reports changes in patient status to the supervising OT
• Educates patients on adaptive equipment and home exercise programs

Certified Occupational Therapy Assistant Requirements & Responsibilities

• Associate Degree in Occupational Therapy Assistant Program required
• Current, unencumbered state COTA License; NBCOT Certification required
• 1 year of previous experience as a Certified Occupational Therapy Assistant preferred
