Job summary

Ramseur Rehabilitation And Healthcare Center in Ramseur, NC is hiring an Occupational Therapist for a long-term care setting. You will perform comprehensive OT evaluations, develop treatment plans, and work with an interdisciplinary team to meet patient goals.

The role requires an OT degree and state licensure with preference for SNF experience. A strong focus on documentation, patient progress, and compliant care is essential for success in this facility.

Qualifications

• Graduate of an accredited university with a B.S., MA, or M.S. or Doctorate in Occupational Therapy recognized by the AOTA.
• SNF experience preferred.
• Holds current license/registration in the state as applicable.

Responsibilities

• Provide comprehensive OT evaluation based on MD orders.
• Document findings on standardized evaluation form timely and accurately.
• Develop comprehensive treatment plan with long/short-term goals, frequency, duration, and modalities.
• Obtain MD orders for evaluations, treatments, recertifications, and discharges.
• Deliver treatment using modalities and adjust as needed while following precautions.
• Document daily participation and progress in progress notes.
• Update treatment plans on a monthly basis and when recertifications occur.
• Document discharge summaries when indicated.
• Complete required documentation and MDS-related items.
• Attend facility and rehab meetings as scheduled.
• Participate in facility committee meetings and rehab team rounds.
• Serve as rehab delegate to the interdisciplinary team.
• Assist in scheduling patients and perform screens as indicated.
• Bill patients accurately for OT services rendered.
• Cover caseload during absences and supervise junior staff as needed.
