Key Rehab is seeking a full-time Program Coordinator (OT or PT) for Macy, NE in a SNF setting. The role involves coordinating therapy services, liaising with community staff, and supporting clinicians to minimize administrative tasks.

The candidate should have a licensed PT, OT, or SLP in the assigned state with CCC-SLP preferred and CPR as required. Prior long-term care coordination is a plus.

Qualifications

• Completion of an accredited PT, OT, or SLP program (Required).
• Prior experience in long‑term care setting and service coordination (Preferred).

Responsibilities

• Serve as liaison between Key Rehab and community staff; coordinate staffing and daily scheduling to meet performance indicators; support clinicians to deliver therapy services.
• Assist in minimizing administrative burden and coordinating with Regional Supervisors to ensure smooth operations.
