Job summary

Key Rehab is seeking a Program Coordinator-PTA to join our team in a skilled nursing facility setting. Primary duties include coordinating staffing and scheduling, serving as liaison between the rehab team and community staff, and supporting clinicians to deliver therapy services.

Required: PTA or OTA license, CPR as required, valid driver's license; preferred: prior long-term care/service coordination experience. Competitive benefits and growth opportunities are offered.

Qualifications

• Education: Completion of an accredited PTA or OTA program.
• Experience: Prior experience in long term care setting and service coordination preferred.
• Licenses/Certificates: Current PTA or OTA license to practice; CPR as required; valid driver's license and insurance as required by state law.

Responsibilities

• Liaison between Key Rehab and community staff; support clinicians and minimize administrative burden.
• Coordinate staffing and daily scheduling to meet KPIs under Regional Supervisor.
