Village Park Post Acute in Eugene, OR seeks a licensed Occupational Therapist to assess referred patients and design individualized rehabilitation plans. The OT will provide direct therapy under physician direction and educate patients and families in therapeutic methods.

Responsibilities include supervision of COTAs and documentation of patient progress. Required: graduation from an accredited OT program, current state license, CPR certification, and preferably 6 months LTC experience.

Qualifications

• Graduated from an accredited school of occupational therapy.
• Current state OT license and CPR certification are required.
• Experience in long-term care is preferred.

Responsabilités

• Assess referred patients and develop treatment plans.
• Provide direct OT services and oversee therapy per physician orders.
• Document patient progress and communicate status to care team and families.
• Refer patients to COTA and supervise staff as appropriate.
• Collaborate with facility staff to integrate therapy into interdisciplinary care.
• Maintain punctuality and reliability for assigned shifts.
