Cascadia EE LLC is seeking an Occupational Therapist for Gabriel Park Post Acute in Portland, OR. The OT will assess referred patients, develop and monitor rehab plans, and supervise COTAs and rehab aides under physician direction.

Candidates should have a degree from an accredited OT program, a current state OT license, CPR certification, and at least six months of long-term care experience. We offer comprehensive benefits and a collaborative team.

Competenze

• Graduate from an accredited school of occupational therapy.

• Current CPR certification.
• Six months experience in a long-term care environment preferred.

Mansioni

• Assess referred patients and develop individualized rehabilitation plans.
• Provide direct patient care and document progress toward goals.
• Refer and supervise COTA(s) and Rehab Aide(s) as needed.
• Collaborate with physicians, therapists, and care teams to optimize patient outcomes.
