Job summary

Village Park Post Acute in Eugene, OR is seeking a licensed Physical Therapist to provide rehabilitative services, restore function, improve mobility, and relieve pain for residents. The role includes developing and supervising treatment plans, documenting progress, and collaborating with the care team to optimize outcomes.

Requirements include a PT degree from an accredited program, current state license, CPR/BLS certification, and preferably six months of long-term care experience.

Qualifications

• Graduate from an accredited physical therapy program with a PT degree.
• Current physical therapist license in the state of employment.
• Current CPR/BLS certification.

Responsibilities

• Establish and administer a treatment plan with therapy goals.
• Assess patient needs and modify care to meet therapy goals.
• Collaborate with all disciplines to plan patient goals.
• Document patient condition and progress per regulations.
• Educate patients and families on therapy methods.
• Communicate patient status to family and care team.
