Job summary

Evergreen Post Acute in Portland, OR is hiring a full-time on-site Occupational Therapist. You will evaluate residents’ functional abilities, design person-centered treatment plans, and deliver therapeutic interventions to improve daily living, mobility, and independence, coordinating with nursing staff and the rehabilitation team to ensure continuity of care.

Qualifications

• Active state licensure (or eligibility) as an Occupational Therapist and graduation from an accredited Occupational Therapy program.
• Experience in skilled nursing or post-acute rehab is beneficial.
• Strong communication and ability to collaborate with interdisciplinary teams.
• Ability to adapt interventions for diverse age groups, including pediatrics.

Responsibilities
