Job summary

Continuum Therapy Partners, LLC is seeking an Occupational Therapist to evaluate, plan, and implement therapy programs improving patient independence. The role includes documenting progress and adjusting plans based on clinical outcomes.

The position requires an OT license (NBCOT) and a Master's/Doctorate in Occupational Therapy, with a preference for 1 year of prior OT experience. Full-time day schedule at Avalon Health and Rehab in Mount Vernon, OH.

Qualifications

• Master's or Doctorate in Occupational Therapy required.
• Current, unencumbered state OT license; NBCOT certification required.
• 1 year of experience preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on outcomes.
• Educates patients and families on adaptive equipment and home safety.

Occupational Therapist Requirements & Responsibilities

• Master's or Doctorate Degree in Occupational Therapy required
