Job summary

Flagstar Rehab is seeking an Occupational Therapist (OT) for a Skilled Nursing Facility setting in New York. The OT will conduct evaluations, develop individualized treatment plans, and coordinate care with a multidisciplinary team.

The role includes supervising COTAs and delivering quality therapy to patients with TBI, stroke, Parkinson’s, MS, cancer, and orthopedic needs. Position offers either W-2 employment or 1099 Independent Contractor options, with weekly pay and performance bonuses.

Qualifications

• Current NYS OT License and Registration.
• EMR competence.
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Malpractice/liability insurance (if applicable).

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for COTAs to follow.
• Supervise COTAs daily.
• Provide quality therapy services to individuals in an SNF setting for conditions such as TBI, CVA, Stroke, Parkinson’s Disease, MS, Cancer, orthopedic surgery, and related diagnoses.
• Monitor and document patient progress using EMR software.

Pay Rate: $46-$48 Hourly ( W-2 ), $57-$60 Hourly ( 1099 )

Occupational Therapist Job Duties:

• Provide efficient evaluations and create individualized treatment plans for COTAs to follow.
• Supervise COTAs daily.
• Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.

Monitor and document patient progress with the use of innovative EMR software.
