Job summary

Flagstar Rehab is seeking an Occupational Therapist to join our skilled nursing facility team in the New York City area. You will perform evaluations, develop and implement treatment plans, and educate patients and families to improve functional independence.

We offer a choice between direct employment or independent contractor arrangements, plus benefits like paid orientation, weekly pay, and flexible scheduling. NYS licensure is required to practice in SNFs.

Qualifications

• Current NYS OT License and Registration.
• EMR Competence.
• State Required Medical Documentation for work in a Skilled Nursing Facility (Physical, TB Test, MMRV Vaccine Record or Titers, Covid Vaccine Documentation).
• Valid Proof of Identity and Work Authorization.
• Malpractice/Liability Insurance (if applicable).

Responsibilities

• Provides Occupational Therapy services to residents by assisting in the assessment and treatment, development and implementation of programs.
• Conducts individual, supervised and group treatment as assigned by the Occupational Therapist and as outlined in the treatment plan.
• Monitors resident's response to intervention and consults with Occupational Therapist if modification is indicated.
• Adheres to department and resident schedule and modifies as appropriate for treatment regimen.
• Maintains acceptable resident care rations as determined by the Occupational Therapist.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Occupational Therapist Job Duties:
