Job summary
21st Century Home Health Services (21HHS) seeks an Occupational Therapist to deliver in-home skilled OT in Sacramento County and surrounding communities. You will collaborate with Case Managers and an interdisciplinary team to ensure safe, functional home environments and caregiver education.
We offer flexible schedules, autonomous practice, and a comprehensive benefits package including 401k with matching, generous PTO, and health coverage. Experience with higher acuity clients is a plus.
Qualifications
- Current unrestricted CA Occupational Therapist License.
- BLS/CPR certification required.
- Valid CA Driver's License and knowledge of local resources.
Responsibilities
- Provide skilled OT services to evaluate and address home-based needs with the Case Manager.
- Address home safety, medications management, wheelchair seating/positioning, and pain management.
- Support falls prevention and management of chronic diseases and dementia for families.
- Act as educator, patient advocate, and member of the interdisciplinary care team.
- Coordinate care and communicate effectively with the care team and families.
- Navigate and support in-home charting and care documentation as part of the team.
