Full Time Occupational Therapist (OT) Home Health

Location: Discovery Bay and surrounding cities.

All positions require field visits to patients’ homes; remote or work‑from‑home positions are not available unless specifically noted.

Responsibilities

In partnership with the Case Manager, you will provide skilled services to evaluate and address the needs of individuals in their homes, enabling them to live safely and productively. Key areas of focus include home safety, medication management, wheelchair seating and positioning, pain and falls prevention, chronic disease and dementia management, and caregiver training. You will serve as an educator, patient advocate, and a member of the interdisciplinary care team.

Qualifications

• Current unrestricted California Occupational Therapist license
• Current BLS/CPR certification
• Valid California Driver's license
• Knowledge of local community resources
• Understanding of human behavior as it relates to illness/injury and the influence of culture on health care
• Knowledge of state and federal home‑care regulatory guidelines preferred
• Experience in home health care highly preferred
• Experience working with high‑acuity patients a plus
• Experience with interdisciplinary teamwork highly desired
• Case management fundamentals highly desired
• Self‑directed, empathetic, critical thinker, outstanding interpersonal skills, leadership skills, organized
• Completion of a computer literacy assessment prior to scheduling an interview

Compensation

$120,000 – $130,000 per year, commensurate with relevant experience. 21st Century Home Health offers generous bonus incentive plans, comprehensive health benefits, and a 401(k) with up to 4% match.
