Job summary

Honey Grove Nursing Center is seeking a compassionate Occupational Therapist (OT) to join our in-house rehab team on a PRN basis. You will evaluate residents, develop personalized treatment plans, and help maintain independence in daily activities and safety.

The OT will collaborate with the interdisciplinary team, adjust plans as needed, and ensure documentation aligns with state and company policies. Texas license required and SNF experience preferred.

Qualifications

• Active and valid Texas Occupational Therapy license.
• SNF or long-term care experience preferred.
• Current knowledge of occupational therapy treatment practices.

Responsibilities

• Consult with patients to evaluate their physical condition and its impact on daily occupations, well-being, and quality of life.
• Assess and interpret patient evaluations and test results.
• Develop individualized treatment plans to maximize ADLs and IADLs.
• Assess patients’ home environments and recommend modifications.
• Provide training on adaptive equipment to support independence and safety.
• Deliver treatments per the care plan and monitor response.
• Collaborate with the interdisciplinary team to optimize care.
• Complete documentation in accordance with policies and regulations.
