Job summary

HealthPro Heritage in Baltimore, MD is seeking an Occupational Therapist to join our long-term care facility's rehab team. The role offers a consistent 30-hour work week in a supportive, collaborative environment dedicated to high-quality patient care.

You will evaluate residents, create personalized treatment plans, educate families, document progress, and advocate for residents to maintain independence. A degree in OT and active state licensure or eligibility are required.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Valid state OT licensure or licensure eligibility.
• Strong clinical assessment and diagnostic abilities.
• Excellent communication and patient-centered approach.

Responsibilities

• Patient Assessment: evaluate physical, cognitive, and emotional abilities.
• Treatment and Education: develop personalized plans and educate patients and families.
• Documentation: maintain accurate records of evaluations and progress.
• Advocacy and Support: help clients access community resources and accommodations.
