Job summary

Seen Health, Inc. is seeking an experienced Occupational Therapist for the PACE program in the San Gabriel Valley (LA area). You will assess participants, design interventions to promote independence and safety, and work within a dynamic interdisciplinary team to deliver high-quality care.

The role emphasizes geriatric/community-based care with strong collaboration across families and providers, including care transitions and home safety considerations.

Qualifications

• Master's degree in OT or equivalent.
• Valid California OT license.
• Proficiency in English and Mandarin and/or Cantonese.
• Strong clinical assessment and treatment planning skills.
• Collaborative work in a multidisciplinary team.

Responsibilities

• Conduct comprehensive occupational therapy assessments evaluating cognitive, physical, and psychosocial functioning.
• Develop individualized treatment plans based on assessment findings and IDT input.
• Provide direct OT interventions including therapeutic activities and adaptive equipment recommendations.
• Collaborate with participants, families, and healthcare professionals to set goals.
• Document progress and outcomes in electronic medical records.
• Monitor response to therapy and adjust plans to optimize independence.
• Coordinate with external vendors and community resources for continuity of care.
• Participate in IDT meetings and share OT insights.
• Adhere to policies, maintain confidentiality, and comply with regulations.
