Optum is recruiting an Occupational Therapist for full-time/Home Health who assesses and plans care for patients, focusing on activities of daily living and home safety. You will collaborate with physicians and the care team to deliver evidence-based therapy and support patient independence.

Eligible candidates must hold current OT licensure in the practicing state, possess a valid driver’s license, and have CPR certification or the ability to obtain it within 90 days.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.
• Demonstrated ability to manage multiple tasks simultaneously.
• Demonstrated ability to work independently.
• Good communication, writing, and organizational skills.

Responsabilités

• Provides service within the scope of practice as defined by the state laws governing Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other members of the health care team.
• Evaluates the patient's functional status and CMS-relevant information to inform the therapy plan of care.
• Observes, records, and reports to the nurse supervisor and physician the patient's response to treatment and changes to condition.
• Conducts patient assessments evaluating function using diagnostic and prognostic tests.
• Function in diverse home situations respecting patient dignity and equality.
