Job summary

Evolve Therapy Services in Greendale, WI is seeking a Certified Occupational Therapy Assistant to support our in-house Skilled Nursing Facility. The role focuses on planning, implementing, and directing OT services to ensure high-quality resident care.

You will screen residents, develop treatment plans with supervision, document and bill per CMS standards, and collaborate with a multidisciplinary team. Requires an OTA license and an associate degree, with ongoing education encouraged.

Qualifications

• Must hold OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Screen residents to determine best intervention or treatment per scope of practice.
• Perform treatment plans and follow short- and long-term goals with supervision.
• Interpret and communicate evaluation findings to residents, families, and team.
• Establish resident therapy schedules in cooperation with team members.
• Conduct therapy per plan and ensure proper documentation and CMS billing.
• Perform additional duties as assigned to effectively treat patients.
• Comply with CMS documentation standards daily with point-of-care documentation.
