Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility partner located in Glendale, WI. This role requires an active OTA licensure and an Associate's degree in Occupational Therapy, with ongoing professional development.

You will collaborate with an interdisciplinary team, screen residents, develop treatment plans, document and bill services, and support residents to reach goals while adhering to CMS standards.

Qualifications

• Active OTA licensure required in practice state.
• Associate's degree in Occupational Therapy required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best interventions.
• Plan and implement treatment goals per plan of care.
• Communicate evaluation findings to residents and families.
