Job summary

Evolve Therapy Services in Glendale, WI is looking for a compassionate Certified Occupational Therapy Assistant to provide in-house therapy within a Skilled Nursing Facility. You will implement treatment plans, document progress, and collaborate with a diverse care team to support residents.

Requirements include an Associate's degree in Occupational Therapy and active OTA licensure in the state(s) of practice, plus commitment to ongoing professional development and CMS-compliant documentation.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screenings to determine best intervention or treatment for each patient.
• Implement treatment plans and follow short- and long-term goals.
• Interpret and communicate evaluation findings to residents, families, and the interdisciplinary team.
• Establish individual resident therapy schedules with the team.
• Deliver therapy per treatment plan and ensure proper documentation and billing.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards on a daily basis.
