Job summary

Evolve Therapy Services seeks a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in New Haven, CT. The role supports resident care with direct OT services in a team-based environment; opportunities exist for full-time, part-time, or PRN schedules.

The ideal candidate holds an OTA license, an associate degree in OT, and commits to ongoing professional development, with adherence to CMS documentation standards and high-quality resident care.

Qualifications

• Active OTA licensure in state(s) of practice.
• Associate's degree in Occupational Therapy.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Perform treatment plans following short- and long-term goals per plan of care by supervising therapist.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team.
• Establish individual resident therapy schedules with team members.
• Conduct therapy according to the treatment plan and document and bill OTA services.
• Comply with CMS documentation standards of practice daily with point of care documentation.
